Is funded by Brisbane South Primary Health Care Network (BSPHN) to deliver Continuation of Supports (CoS) to CALD participants in the Brisbane South region. The CoS program is for those individuals who previously accessed support under Partners in Recovery program the Day to Day Learning program, the Personal Helpers and Mentor service and have been found ineligible for the NDIS continue to receive appropriate support using a recovery and strengths-based framework.
CoS program will provide assistance in the following areas:
Assisting you to find and link in with services that best support you (e.g. Clinical health and mental health services, cultural supports, employment services social groups, housing services);
Supporting you to build self-confidence and self-efficacy (e.g. provide individual supports such as information, advice and referral, supports navigating the service system, attending critical appointments and collaborating with service support to avoid duplication); and
Helping you to understand how different service systems work (e.g. collaborating with the broader mental health sector to increase access and equity of services for CALD individuals, build capacity of mainstream service providers to work more effectivity with individuals form CALD background.)
